This project will review and assess the current state of several important Patient Quality Indicators which  Nurses have significant impact and control over, including falls, Wound/Skin Care, Delerium, and Mobility. The  student will complete a review of any current relevant policies and supporting documentation related to Best  Practice for these indicators, including resources from the CNO and RNAO, with each student taking the lead  on one or two of these topics. The ultimate purpose of the project will be to have the students understand the  important role nursing plays in these indicators, the impact they have on patient safety and outcomes, and the  improvement of these metrics within Royal Victoria Regional Health Centre. 


*to note a similar request has been entered by Stevenson Memorial Hospital and if partnership continues this  project could be done collaboratively across both hospitals* 



Livrables

The following outcomes will be achieved for each of the metrics being explored:  

• An assessment of current state, with a review and understanding of how these indicators impact  patient quality care 

• Recommendations for improvement of the metric  

• Development of an education plan for sharing this information with the clinical units, including the  importance of the indicator, current state, opportunities for improvement, and how the improvement of  these indicators will impact both patients and nursing.  

• Development of a ‘connecting the dots’ process that weaves all of the impacts together as they feed  into each other and the improvement of patient care. 

• Evaluate any additional needs to help augment care and data extraction from the hospital EMR • The RVH team will provide them with current metrics and support them in setting a measurable  improvement outcome for each indicator.  

The organization will benefit from an ultimate improvement in patient safety and quality care as it  relates to these factors. The students will benefit from the development of an understanding of both  the importance of, and the role nursing and other disciplines plays, in these patient safety outcomes.  They will also benefit from an understanding of how acute care centres collect and track data, and  working with Clinical Educators and Professional Practice Leaders to plan out how this type of  education can be delivered to nurses and allied health on the clinical units in order to ensure  engagement. 

4. Student Involvement: 

The students will work onsite and collaborate with members of the Professional Practice, Quality and Risk, and  clinical teams.  


5. Resources Required (if any): 

The students will be provided with access to overall data on patient outcomes, a network login, and EMR  access as required in order to accurately complete this project.  


6. Support Offered to Students (i.e. weekly meetings, mentorship, etc.): 

Students will be mentored by the Professional Practice teams, including set weekly meetings which will serve  to mentor, evaluate progress, and set action items and next steps. There will also be ongoing availability of  mentorship and support outside of these meetings.
